Overview

In a safe and respectful manner, provides a variety of supportive services, as outlined in the care plan, which serve to assist an elder to remain safely in the home. Responsible for maintaining consistent communication with the coordinator through regular consultation, verbal and/or written reports

Specific Responsibilities

  • Immediately notifies the coordinator of any changes in the work schedule
  • Maintains the confidentiality of the client at all times
  • Consults regularly with the coordinator about changes in the client’s condition or environment
  • Follows the care plan of supportive services as outlined by the Aging Service Access Point (ASAP)
  • In a compassionate and understanding manner, assists the client with household management
  • Shops for the client’s groceries, as outlined and authorized by the ASAP
  • Performs light housekeeping tasks and routine kitchen and bathroom cleaning
  • Vacuums and empties trash
  • Does the client’s laundry and changes the client’s bed linens
  • Plans and prepares meals for the client as authorized by the ASAP
  • Assist the client or primary caregiver with bathing the client
  • Provide skin care to the client through the application of over the counter lotions that have previously been used by the client without adverse reactions.
  • Provide shampoo and hair care to the client
  • Shave male patients with the use of electric razor only
  • Assist the client and/or primary caregiver with dressing
  • Assist clients and/or caregiver with the application of dentures, hearing aids, braces, splints, or prostheses
  • Assist clients with the elimination pattern by use of bathroom, commode, bedpan or urinary drainage bag
  • Transfer clients who can assume 50% of their body weight
  • Assist clients with ambulation and/or encourage an exercise program
  • Prepare and serve meals to a client and assist with feeding if outlined on the care plan from ASAP
  • Remind alert and responsible clients to take medications, but never administer
  • Report changes in client’s condition to the Nursing Supervisor or Coordinator
  • Attend 6 hours of in-service training a year
  • Maintain a neat, clean, and professional appearance
  • Provide tasks in a time effective fashion
  • Accepts care plans and task lists for clients who may be receiving clustered services
  • Attends required MRHHC in-service training programs to meet required training hours
  • Presents a neat, clean appearance at all times

Requirements & Qualifications

  • Successful completion and documentation of the 17-hour personal care training course or home health aide or CNA certification. Completion of the three-hour orientation program containing company policies including OSHA.
  • Ability to speak, read, and write basic English
  • Must be at least 18 years of age
  • A copy of a recent physical and TB test result showing good physical health according to all applicable regulations
  • Minimum of one year experience with cleaning in a home or in a work setting
  • Proof of eligibility to work in the United States

Physical Requirements

Office Work

  • Exerts up to 10 pounds of force occasionally, and or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects. Sedentary work involves sitting most of the time. Jobs are sedentary if walking and standing are required only occasionally and all other sedentary criteria are met.

Patient Visits

  • Exerts up to 50 pounds of force occasionally and/or up to 20 pounds of force frequently and or up to 10 pounds of force constantly and a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body.

Visual Acuity

  • Must have visual ability to prepare and analyze data and figures; transcribe; operate a motor vehicle; read extensively; operate machines, tools and utilized devices.
