We are seeking LPNs to work with our clients in the Ashland, Natick, Newton and also our Boston areas and neighboring towns.

     

     

    Under the direction of the RNs, LPNs establish effective relationships with the patient, family and health professionals and makes a commitment to:

    • Collect health assessment data to determine significant issues
    • Collaborate with health professionals to assess, plan and implement patient plan of care
    • Develop discharge plans in conjunction with the patient, caregivers and other health professionals to meet desirable and attainable out comes
    • Show flexibility with day to day scheduling to adapt to case load needs

    Minimal Qualifications:

    • Current licensure or permit by the applicable state board of nursing
    • Associates Degree
    • 1+ years' experience in a clinical setting
    • 1+ years experience in homecare
    • Driver’s license and reliable transportation
