To provide medically necessary evaluation and treatment of individuals following accepted practice guidelines, in a home health care setting. The Occupational Therapist is responsible for developing and maintaining skills necessary to provide the highest standard of patient care; competency in provision of skilled therapy services; teaching patients/caregivers/staff to carryover non-skilled interventions; maintaining relationships with all members of the health care team; evaluating the effectiveness of treatment rendered; and documenting patient care accurately and in a timely manner.
Assist the physician in the functional evaluation of the patient and in the development of the individual plan of treatment, including both short and long-term goals.
- Examine patients and patient history, review of systems performing appropriate tests and measure to identify potential and existing problems.
- Establish therapy diagnosis, prognosis, and plan of care based on examination findings
- Establish attainable goals and discharge plan in collaboration with patient and caregiver
- Provide skilled intervention to assist patient in reaching attainable goals
- Re-evaluate patient at appropriate intervals and modify treatment/goals as needed
- Establish and maintain positive rapport with patients, caregivers and health care team
- Communicate regularly with healthcare team to ensure high quality patient care
- Provides evidence-based clinical care for home health nursing practice utilizing the ANA Scope & Standards of Home Health Nursing Practice
Requirements and Qualifications:
- Current licensure or permit by the applicable state board of therapy.
- Thorough knowledge of professional theory, practice, techniques, principles and equipment.
- Effective oral and written communication skills. Ability to document patient care effectively. Ability to establish and maintain a working relationship with patients, other health care professionals, physicians, and the public.
- A minimum of one (1) year’s experience.
- Must hold current state licensure, CPR certification, driver’s license, and automobile liability insurance.
- Exerts up to 10 pounds of force occasionally, and or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects.
- Sedentary work involves sitting most of the time. Jobs are sedentary if walking and standing are required only occasionally and all other sedentary criteria are met.
- Must have visual ability to prepare and analyze data and figures; transcribe; operate a motor vehicle; read extensively; operate machines, tools and utilized devices.
